What’s the reason(s) it isn’t being used and why isn’t it required?

You have to ask the teams.  Ironically teams asked for a way to stop canceled games.  KYMSFA got the KHSAA to create middle school game contract form, no one uses them.

I am back to the question of why isn’t it enforced that teams sign the contract when they schedule the game, especially the teams that pay the money to be part of the association?

Oversight for regular season events is at the local school level. The same entities that wont sign contracts.
